NHS Health Checks Suffolk

The NHS Health Check service is offered in Suffolk via GP Fed to pharmacies in geographically deprived areas.  This has been subcontracted by Suffolk Public Health under the Primary Care Contract for NHS Health Checks delivery.

The NHS Health Check is for adults in England between the ages of 40 and 74 and is a screening service designed to predict the 10-year risk of a patient developing heart disease so that they may be offered lifestyle advice and intervention where necessary.

NHS Health Checks are offered to Suffolk residents or those registered with a Suffolk GP. Invitation letters are sent to patients and they can also have their health check opportunistically.

If you are interested in offering NHS Health Checks in your pharmacy, please email sgpfed.healthchecks@nhs.net and we will introduce you to the commissioner. Who will cascade details of areas of need Contractual information and how to sign up if the contract has capacity.

Training Requirements

GP Fed will support sub-contractors and their support staff ensuring that all staff providing a health check meet the current workforce competency requirements as outlined in NHS Health Check Competency Framework, (PHE, July 2020), and subsequent updates to the Framework. This could include both face to face and online provision of training materials.

The Suffolk NHS Health Check reporting template form is available on Pharm Outcomes and must be used to submit the health check and receive payment.

Contractual information, standard operating procedures and resources can be requested from GP Fed.

All patients should receive an NHS Health Check results card during their appointment. The service has be configured to generate a patient information leaflet that is pre-populated with patient specific data captured during service delivery.
You will be prompted to print/save and send the leaflet when you save this provision. 

Alcohol Screening

Many pharmacies already include alcohol advice in their Health Checks but this is officially part of the check. You will find the full Audit-C questions in the service template.  AUDIT-C and Full Audit Questions.

Dementia Screening

A requirement for the NHS Health check is to offer appropriate information on dementia to patients between 65 and 74 years of age.

Reporting of NHS Health Check results is now through Pharm Outcomes details must be entered on the online reporting system to receive payment.
